Tonight on 20/20... RAPS: # ...got my other chick, hugging wit my other chick, hugging all night... They're the best in the world at what they do. The number-one megacrew in the world rapping from NZ. RAPS: #....you know it... # We meet the master behind the moves. Everything I've learnt in hip hop is self-taught. Now the crew's going for Vegas gold again. We need fresh meat ` fresh meat in the studio. (CHUCKLES) How young is too young for cosmetic surgery? WOMAN: When Charm was 10, she was already unhappy with the way she looked. And what if your own daughter asks you for a breast enlargement? Be nice to me, Dad. Being in practice for 30 years, I felt, 'Who would give them the time and the patience that their father would give them? Also, Joan Rivers talks life and liposuction. What does your grandson call you? What does your grandson call you? He calls me Nana New Face. (CHUCKLES) She reveals her motivation... All I want them to say about me is, 'You look amazing for your age.' ...and what procedures she wouldn't do. Have you had your vagina tightened? Have you had your vagina tightened? Why bother? I'm Sonia Wilson. If you take a bit of Polynesia, add a bit of swag, you've got Polyswagg ` the style of hip-hop dance straight from NZ that's seriously dominating the international stage. Paris Goebel is the queen of swag. She's just 20 years old, and she's a true NZ success story. Her dance moves and choreography have her hip-hop crews ranked number one in the world. They've appeared on American reality TV. She's flown around the world to teach her style of dance, and now, as you're about to see, it's time for this year's auditions to join Paris' royal family of hip hop. Erin Conroy finds out who's in and who's out. HIP-HOP MUSIC MAN: # Rack, rack, rack city. PARIS: Dancing's probably the best feeling in the world. It's almost like everyone vanished off the face of the earth. # Ten, ten, ten twennies and them fifties, chick. You're the only person left. # ...no guest list. And the music. # Got my other chick, hugging with my other chick... Every move you see is 100% her inspiration. Dance is what she does for fun. It's her livelihood. It's her life. And over the next few days, life is gonna get a little bit crazy for this woman. # Rack city, chick. Rack, rack city, chick. # So, auditions are four days long, and it's so hard. It's very challenging emotionally and physically, but for us` It's important for them to show us that they have what it` what it takes and that they're gonna work hard enough to get this. The beauty of it is if they make it, they actually have the opportunity to chase their dream. MAN (RAPS): # That's mine, the heart beats... # South Auckland. In the middle of the industrial zone is Palace Studios. Auditions are about to get under way for the Studio's 2012 hip-hop crews. # Tellin' me I'm invincible. # I'm invincible. # I'm invincible. Get a reaction like this to your dance here, and you can be quite sure you're heading for big things,... # I'm invincible. # ...because if you're picked, you'll be studying under one of the most highly recognised hip-hop dancers and choreographers in the world right now ` Paris Goebel. # Oh, oh, # oh... I've always danced ever since I was, like... As soon as I could walk, I was dancing and performing. I told my mum, like, 'I wanna start` start dancing.' # I've been roaming around; I was looking down... # She took me to dance school. She enrolled me into everything under the sun. Like, I had to do jazz, ballet, tap, musical theatre. She didn't last long at any of these, but one hip-hop class at age 11, and she was hooked. UPBEAT HIP-HOP MUSIC I love this stuff, and that was, like, the beginning. The beginning of her own NZ take on hip hop. What's Polyswagg? It's our swag and our Polynesian. It's, like, put together. How we dance is unique to us, because we're from NZ, and it's kind of our heritage and culture and the way we're brought up. It's like that strength as a woman mixed with hip hop that allows us to create this, um,... amazing presence on stage that people can't put their finger on. So, are you actually a trained dancer? No. I would have killed to have technique and, um, a background of training, but I don't. Like, everything's... Everything I have learnt in hip hop is self-taught, so... Well, you seem to have done quite well out of it. Well, you seem to have done quite well out of it. I'm trying to. I'm trying to do OK. 'OK'? Understatement. # Who's in charge of the girls? # Paris formed a crew made up of just her friends. They called themselves Request, and they did much more than just OK. TRIUMPHANT MUSIC I, kind of, set a goal in my head to make Request huge ` like,... and do something great. Yeah. and do something great. Yeah. < Did you? and do something great. Yeah. < Did you? I think we did. (LAUGHS) BEYONCE'S 'GIRLS WHO RUN THE WORLD' BEYONCE'S 'GIRLS WHO RUN THE WORLD' # Girls, we run this mother... # In fact, Paris' crews have racked up an impressive trophy cabinet. She and her girls have flown around the world,... Hong Kong airport. We're in Rome. ...invited by international studios wanting her to teach them her style of dance. They've even appeared on the reality show America's Best Dance Crew. FUNKY MUSIC And every year, she's entered crews in the World Hip-Hop Champs in Las Vegas. She's walked away with medals. RIHANNA'S 'S&M' Last year, she entered four teams, winning three golds and a silver. The number-one megacrew in the world. Varsity champions 2011. I wanna show off our gold medals. ALL: NZ! So, three golds and a silver last year. Yeah. Yeah. What can possibly be left to achieve? Gold in all. (LAUGHS) UPBEAT MUSIC It's day one of new crew auditions. Just welcome everyone to the, um, Palace auditions for 2012. Give yourself a round of applause. You're in the right place. Dad Brett arrives and sets the standard for this year's intake. Right now, this is the number-one studio in the world, without a doubt. I told you` (EXCLAIMS) From more than 100 auditioning, three crews of eight will be chosen. Bubblegum ` the junior team; Sorority ` intermediates. The new adult crew is Misfits, and their large group crew ` the Royal Family. After walking away from last year's Worlds with a gold medal around her neck, Samia (10) is back to audition again for a place in the junior crew. My name is Samia, and I've come to audition for Bubblegum. I'm feeling a little bit nervous, but really excited, and I'm hoping I get it. This is my best friend, Brodie. Hi, Brodie. > Hi, Brodie. > Hi. I wanted to audition cos I wanna be dancing with the best. With the hope of dancing together, these besties have been practicing all year ` practicing anywhere and everywhere, as well as travelling the two and a half hours from Whangarei,... It's 165km. ...to get to rehearsals. NICKI MINAJ'S 'ROMAN IN MOSCOW' I told you... (EXCLAIMS) Big hit. Pull. All the dancers will be tested with three difficult routines to learn and perform each day. Shot, guys. When it comes to the auditions, what are you looking for in a dancer? The most important thing that we look for is heart. That's... That's, like, the number-one thing. I mean, some kids don't pick up fast and they... aren't the best in the audition, but if I see that they are working so hard and that they love to dance, I think I would take that over someone that has all the talent in the world but no commitment. So for me, the most important thing is heart ` just having a love for dance. Most of your crews up until now have been all female. Why is that? We started as girls just cos you just want to dance with girls, you know, but it ended up turning into something, like, we wanted to prove ` I just wanted to make sure that, you know, we showed other girls that you can` you can do something great and you can go up against the guys. So for this year, are boys out of the question? So for this year, are boys out of the question? No. No way. Oh my gosh. No way. If anything, I hope boys come and audition. Like, we need fresh meat ` fresh meat in the studio. (CHUCKLES) Five, six, seven, eight. Boom. (SPEAKS INDISTINCTLY) UPBEAT MUSIC Paul (17) is auditioning for the adult team, Misfits. I love dance. It's what I do in my spare time. Without dance, I feel, like,... depressed. (CHUCKLES) # They all from Louis, Louis. Get 'em wetter than scuba divers. # If I have my name read out that day, I will be so, so over the moon, so excited that all what I've been looking forward to has actually come true. Yeah. My heart will be beating. Dancing since he was 5, his first love was tap and ballet, but since he found hip hop, he's been loyal to his all-boy crew ` until now. It was a pretty big decision. It was pretty hard for me to decide between the boys, who I'd been with for how many years, and to come and join a new crew, it was` it was a pretty hard decision. Day two. Here we go. UPBEAT MUSIC Day by day, each dancer is closely assessed. Some more sets, more energy. Just have fun and` and try our hardest. Different combinations are tried. Don't be shy. Just be hungry for your spot. So it's, like,... (IHNALES) choke, breathe,... (INHALES) boom. If I compare it to the auditions last year, there's so much more talent and, like, I guess, like, fresh meat in the room, so it's awesome. When she's ready, she'll start reading out names for you to come out to audition. You're not ready if you're standing there watching. FUNKY MUSIC And as the days go on, the pressure to perform never stops. FUNKY MUSIC When you see them performing their hearts out in the auditions, you have quite a reaction. (LAUGHS) (LAUGHS) What's going on there? Dancers are really verbal, so it's hard for us to contain ourselves, so we're, like, 'Oh!' or, like, jumping up and down or clapping and screaming, because I think in the hip-hop culture, it's very` very expressive, so if you see someone, it's really important to show your appreciation. On the morning of the final day of auditions, Paris and her girls are narrowing it down. ALL: No. Yep. Maybe. She's a` sometimes a distraction. Yes, yes. What lies between this and being named in the crew is one final solo dance ` the last chance to prove yourself. Think you're all on the same level, and your solos ` will they push you up, down or just keep you there? So just make sure you guys kill it. After the break ` big moves ahead for this 20-year-old,... I discovered that sometimes I'm on the go so much that I don't do anything for myself, and I think this is something I need to do for me. ...and the life-changing solos for brothers Mike and Joe. CHEERING . Welcome back. In part two of our story Polyswagg, it's make or break for our hip-hop hopefuls. Stand by to see who's in and who's out after a gruelling series of auditions, and Paris reveals her plans for the future. It's decision day here for the 100 hopefuls, and a final solo dance to convince Paris you've got what it takes. UPBEAT MUSIC The pressure's on, and from nowhere comes brothers from Tokoroa Mike and Joe. HIP-HOP MUSIC, CHEERING It's a small town. There's no opportunities, or less opportunities. Dance isn't very big there. # Yeah. Stay prepared for... # I work at Tokoroa New World. Um, I've been working there for six years now. $1.18, OK? You know all the customers there, and it's a friendly place to work in. Thank you very much. I get told off putting out the ham or filling the salads or doing the chickens. I am doing a bit of movement and some dance on the side,... and I try not to get in trouble. Get a few weird looks from the customers? Get a few weird looks from the customers? I do, sometimes. Yes. I have a passion for hip hop, and growing up in a town that doesn't embrace dance is challenging. 20-year-old Mike and his younger brother have always danced together until Joe moved to Australia last year. If we do get in, we're willing to just drop everything and move to Auckland. Mike's never lived away from his family, but the brothers will do whatever it takes. For you and your brother, Joe, what would be your ultimate dream come true for your future? Backup dancing for Britney Spears. Backup dancing for Britney Spears. You and Joe as brothers? Yeah. That's` I think that's our most ultimate dream is to backup dance for a famous artist, whether it be Britney Spears, Beyonce, Janet Jackson. < And Mike and Joe. < And Mike and Joe. And Mike and Joe, yeah. And for it to play back here on TV and for our people to know who we are and be, like, 'Hey, they're from Tokoroa,' and for us to know that is the most ultimate dream come true. SOFT MUSIC How hard is it to read out the names of those who are successful and let down those who aren't? I get really nervous, because I hate telling people that they didn't make it. For me, it's, like... (INHALES) For me, it's, like... (INHALES) That's why your dad reads out the little ones? Yep. I always tell my dad, like, 'Can you read it out?' I-I guess the reality of auditions is not everyone can make it, and that's... that's the hard part. So, in no particular order... The moment has arrived. First to be announced is Bubblegum. With another gold medal around her neck will be Samia. And also into the crew will be Brodie. (CHUCKLES) (CHUCKLES) Uh, Brodie, congratulations. You have worked your butt off. I'm really excited, cos now I get to go back and defend my title of winning the World Hip-Hop Champs, so can't wait. Really excited, and, yeah, just happy. Crew by crew, they're called. ALL TALK AT ONCE Next, who's a fit for the Misfits? The world's waiting for something new to hit the stage, and Misfits is it. Third spot, which I'm very excited about, uh, goes to Paul. Third spot, which I'm very excited about, uh, goes to Paul. APPLAUSE, CHEERING # Just stay there... # I feel amazing. I never thought this could happen, but, yeah, it's happening. And the megacrew the Royal Family. Fourth spot goes to Mike. CHEERING, APPLAUSE Sixth spot goes to... Joseph. CHEERING, APPLAUSE Joe never really put up a fight for his spot. He was always just kind of chilling, and, um, when he did his solo after Mike, he... he showed us. Like, he was, like, 'This is my spot.' It was, like, 'Yeah. He deserves it,' and he just` he really put his heart on the line, and that for me made my final decision on him. One, two, three. One, two, three. ALL: Royal Family. It's 'goodbye Tokoroa, hello Vegas' for these brothers. I think it's just time for us to move on. I think it's just time for us to move on. Yeah, to move on. Take a new chapter in life and... take this opportunity, run with it, go to Vegas. (CHUCKLES) UPLIFTING MUSIC What's it like to see someone being given these opportunities through something that you've provided? I think the best word is 'rewarding', and, you know, it's not even them coming up saying, like, 'This is so much fun.' It's how they act and dance that you can just see right through their souls that this is what they wanna do, and if they weren't doing this that they would be completely lost, you know, so it's really cool that they've found something that they can... that they can shine, and they can` and they've also found a place that they can call home. That's really cool. POIGNANT MUSIC It's Saturday morning, the first weekend since the crews were named. Everyone else is sleeping. It's an early morning with that camera, brother. It's too early for the sun to be up, but with world champs just months away... There's not one other crew in NZ that is currently up training, and that's how we become world champions. We work harder than anyone else in the world. ALL: Family. Training for real has begun. INSPIRATIONAL MUSIC ALL: Go, go. Push. Last year, we won three golds, one silver. This year, we're gonna go for four golds, and you're gonna do it. It starts today. It's one step closer to achieving the goals of her dancers. Las Vegas in August. We will be gold medallists. But for Paris herself, she too is making moves. Do you see a time with your dance that you're gonna need to move on from NZ? Oh yeah. Definitely. I think I've always had that itch inside of me ever since I was younger. I discovered that sometimes I'm on the go so much that I-I don't do anything for myself, so, yeah, um, I talked with my parents, and we` we kind of put a date in place that in May I-I'll move to LA. This move to LA. What's that for you? It's my door-opener. It's an opportunity for me. It means no excuses, cos when I'm in NZ, I-I find, 'Oh, I would do this, but I can't because I'm here.' 'Oh, I would audition for this. I can't because I'm in NZ.' 'Oh, I'd be great for this part, but I'm all the way over here.' So for me, it means no excuses. There's no excuse for me to not achieve in this. There's no excuse for me to not be chosen. No excuse for me to meet the right people. There's no excuse, because I'm there, so for me, it's` it's, like, finally I can` I can push and I can do, and if things don't move as fast, that's OK, but I know for me there's no excuse for me to not be as motivated as possible to, I guess, achieve what I want to. No excuse. No excuse. No excuse. ...not to live your dream, to achieve your dream. Yeah. Pretty much. Yeah, that's perfect, actually. POIGNANT MUSIC Now, since we filmed that story, Paris has received yet another accolade. She won female choreographer of the year at the World of Dance Awards in America. And, in the Auckland Regionals last weekend, all her crews placed first. Nationals are in April, where they'll qualify for the Worlds. Good luck to them all. Uh, next on 20/20, the cutting edge. Should fathers be performing cosmetic surgery on their own kids, and how young is too young? From a very young age, I really wanted to get it done, so for me, it wasn't a big deal to get a breast job when I turned 18. Why? Just to build my self-esteem, and I didn't have large breasts when I was young, and my friends did. Some may question a father doing surgery on his own daughter. > Isn't that a little daunting to operate on your 18-year-old daughter? . Welcome back. Americans are being cut, stretched, tucked, smoothed, tapered, injected and filled to enhance their appearance in record numbers. Almost 14 million cosmetic plastic surgeries were performed last year alone, creating a $10 billion-a-year industry. Tonight, Barbara Walters asks, 'How young is too young and how old is too old?' We also meet a surgeon performing work on his own daughters. We need to get the crow's feet. Who's next? Preventative, preventative. Make me look young. Plastic surgeon Dr Michael Niccole has two adopted daughters, Brittany,... I want you to do a little here so it lifts. ...and Charm. OK. OK. Dad knows best. When Charm was 10, she was already unhappy with the way she looked. Charm, you had plastic surgery when you were 10 years old. > Charm, you had plastic surgery when you were 10 years old. > Belly button. On your belly button? On your belly button? Yeah. On your belly button? Yeah. Was it out, and you made it in? Yep. It was an outtie, and I didn't like it at all, so I went under anaesthesia and he changed it for me. And when Brittany turned 18, she got the surgery she had wanted for years, and again Dad performed the operation. You wanted to have your breasts made bigger? Yes. From a very young age, I've really wanted to get it done, so for me it wasn't a big deal to get a breast job when I turned 18. Why? To build my self-esteem. I didn't have large breasts when I was younger, and all my friends did. Some may question a father doing surgery on his own daughter. > Isn't that a little daunting to operate on your 18-year-old daughter? Not really. Being in practice for 30 years, I felt who would give them the time and the patience that their father would give them? DANCE MUSIC PLAYS, WOMEN CHEER Brittany, Charm and their friends, most still in their early 20s, have already had many beauty treatments, and they are still looking forward to a lifetime of cosmetic enhancements. How do I look, Dad? How do I look, Dad? Sexy. The girls and their friends have had other cosmetic procedures ` lip injections, Botox, skin smoothing and tightening. Next. Cosmetic procedures are rapidly growing among teenagers from 15 to 19. I have had my nose done, uh, when I was 21. We'll put a little more on this side than this side. I was thinking about getting my breasts redone a little bit bigger. You don't need much. And then I get Botox now. At 23? > At 23? > Mm-hm. Really? > Really? > Yes. You're going to get radio-frequency stimulation, plus light stimulation. I get Botox as well and lasers and peels for my face. Fine little needle. This is 20 units of Botox. Be nice to me, Dad. Be nice to me, Dad. I will. I also get it in my armpits so I stop sweating, which is` which is nice. There are people who will look at these two beautiful girls and you, Doctor, and say there's just too much emphasis now on looks and looks. At 23 years old having Botox and` and fillers. It's just sort of wrong that we place this kind of emphasis. > I` I somewhat disagree because I'm not changing their looks in any means. They want maintenance. They don't want to get old. Uh, they wanna stay young. What's too young? What's too young? I think 18 is a good age. 'Beauty and cosmetic medicine' expert Marie Czenko advises against most cosmetic surgery while still a teenager. Don't do something permanent to your body at that young age unless truly there is the absence of a normal development or opportunity for a normal development. Your hands look good. Dr Nicholle also has the distinction of treating one of the oldest cosmetic-surgery patients. BIG-BAND MUSIC In her day, Marie Kolstad had no notion of getting breast implants. But then at 83 years old, with four children, 12 grandchildren and 13 great-grandchildren, Marie, who works as an apartment manager, decided it was time for breast implants. Why did you want to do this? Barbara, I've been in business for a long, long time with a lot of, uh, competition from younger people, and part of it is, for a woman, having good posture and nice breasts and giving off a good appearance so that I can meet with the competition. Then 83-year-old Marie Kolstad came in and said, 'I want bigger breasts,' right? Bigger and perkier. A lot of this has to be up to you. Marie was heading into unchartered waters. No one has studied the effects of implant surgery on the elderly. Any time that you're having cosmetic procedures, whether you're 84 or 24, your overall health is the most important factor. We had a cardiologist go through her impeccably, and he said she's` she's young, she's a 50 83-year-old, so why not? What did your grown children say? They were afraid of me going under the anaesthesia, so they weren't that happy about it. But I didn't tell them till the day before. Two weeks after the surgery, Marie was on the beach in Hawaii, sporting a new figure and a new outlook. Are you happy with what you did? > Are you happy with what you did? > Yes, I am very happy. UPBEAT MUSIC In fact, Marie was so happy that she signed up for several other cosmetic procedures, including a nose job. She had a collapsed nose, so I did her nose. I did fat transfer to the cheeks, the nasolabial folds, upper eyelid. Youth is puffy. Youth is full. And I put some fat in her temporal area here too. Well, you look great, Marie, and your attitude is great. And I feel good. I don't say to every woman my age, 'This is for you.' And so I'm out there to find my place in life and to continue to, uh, take care of myself. Six, seven and a-one... People think that when you turn 80, you're old. I didn't give that a second thought,... Now come right around. ...because I don't feel old and I don't think old. I think the same way as I did 30 years ago. Incredible. Next on 20/20, the queen of plastic surgery and comedy, Joan Rivers. My mother used to look at me and say, 'Looks don't count.' Now get out of my sight, you big lump. What we all love about you is that you do make fun of yourself. What are some of the things you say about Joan Rivers? Uh, with facelifts? every time I cross my legs, my mouth snaps open. Every time now that I swallow, I have an orgasm. I mean, I have a million jokes. . Welcome back. At 78 years old, Joan Rivers is the queen of facelifts. Tonight, Barbara Walters asks her how many, why, and in some cases, why not? The answer to that one might shock you. It's a revealing look into the personal life of a comedy legend. UPBEAT MUSIC ...again. Joan Rivers, who is now 78 years old, has become almost as famous for her facelifts as for her stand-up comedy. Don't you think men like intelligence more when it comes down to it? Oh... For Joan, it has always been about looks. I mean, it's a brain, you know` No man has ever put his hand up a woman's dress looking for a library card. I'm sorry. In her 30s, obsessed about how she looked on the Johnny Carson show, Joan got a surgical eyelift. Little Joan Rivers. Her routine has always included poking fun at her obsession with her own appearance. My mother used to look at me and say,... (YELLS) 'Looks don't count. 'Now get out of my sight, you big lump.' 'Now get out of my sight, you big lump.' LAUGHTER What we all love about you is that you do make fun of yourself. What are some of the things you say about Joan Rivers? Oh, with facelifts? Yeah. Yeah. Every time I cross my legs, my mouth snaps open. Um, I really feel I've got my face tightened, because now every time now that I swallow, I have an orgasm. I mean, I have a million jokes. But, you know, by laughing at it, it makes it OK. What does your grandson call you? What does your grandson call you? He calls me Nana New Face. (CHUCKLES) And when he was younger, the joke was he had never seen me without bandages, so one time he saw The Return of the Mummy, and he ran to the TV set and he went, 'Grandma. Grandma.' and he ran to the TV set and he went, 'Grandma. Grandma.' BOTH LAUGH Why are you so open? You didn't have to be. Because we're in a society where looks count, Barbara. That's number one. And all the beautiful women lie and say, 'I've done nothing,' and that is so unfair. And I just wanted women to know it's OK. Do it. Joan was indeed transformed over the years. As she looked younger and younger, she made more and more fun of her appearance. Recently, she joked that she has had 784 cosmetic beauty procedures. How many have you had done? How many have you had done? I've had two facelifts. Real facelifts, you know. And then` And then a little bit here` And then a little bit here` A little there, a little here, little there, a lot of fill. I do fill every six months. I do fill every six months. Well, you and Melissa` I do fill every six months. Well, you and Melissa` Tweaks. Tweaks. Uh, it's like if you have a car, you do a little tune-up all the time. Do you do other surgery with the rest of your body? Uh, I did two surgeries. I did one ` my breasts were too big after Melissa. I had them made smaller. And then I got a tummy tuck. How do you choose a doctor? The first thing you do is ask your hairdresser because they see the` the work. They see the scars. They see everything. And then look them up on the internet. Check out if they've been sued. And then interview them cos you need someone with, uh, compassion and human` human feelings. There's enough here to be a skin donor. Joan even devoted the season opening episode of her reality show, Joan & Melissa: Joan Knows Best, to her latest cosmetic surgery. Should I get a facelift? Look at that. You could make a purse. Look at that. You could make a purse. LAUGHTER You look wonderful today. She revealed the entire process, beginning with plastic surgeon Dr Norman Leaf. There's a little bit of fullness here which I think hangs down a little, especially... And Melissa revealed deep reservations about her mother going under the knife yet again. Seriously, can we discuss the risks? I'm very worried. Melissa felt there was danger, and she didn't want you to do it. And there is danger. Anaesthesia is very dangerous. I always interview the anaesthesiologist. I will not have a nurse do it. Uh, I was very careful. I picked an amazing doctor. I picked a great anaesthesiologist. We're probably gonna do the laser-tip lipo, which is great for this area. You are very courageous on your reality show. No make-up ` you show yourself. No make-up ` you show yourself. No make-up. No make-up ` you show yourself. No make-up. Bandages ` you show yourself. Let's go. You don't have to do that. Why'd you do it? Cos I think I wanted people to see what it really was about. It's water. It's water. Yes, it is water. Ice for your chin. Eat your jello, please. I look like a nun. Our Lady of Lidocaine. So, is it worth it just to have a tighter neck to go through all this? So, is it worth it just to have a tighter neck to go through all this? Uh,... in my case, yes. (LAUGHS) Why? Why? No, I think it gives you such self-esteem. It makes you feel good. It's a very happy experience. All I want them to say about me is, 'You look amazing for your age.' You look amazing for your age. You look amazing for your age. That's all you` That's what plastic surgery's about. What do you say to people who say, 'Joan Rivers, you have just done two much'? And I say, 'Mind your business.' And if it's a man, I always look at the wife, who's usually 11 years old with fake breasts. (CHUCKLES) OK. And what's the most extreme kind of plastic surgery that you've heard of? Oh, uh, vaginas now. What? What? They're tightening their vaginas. Well, vaginas can get loose. I see. I see. And, uh, if you've had a couple of children and, you know, and you're in an earthquake and you suction yourself suddenly to the floor, time to tighten. They actually take skin and tighten it and make it smaller and` and tighter. I never thought I would ask you this question, Joan. Have you had your vagina tightened? Why bother? Why bother? BOTH LAUGH Why bother? BOTH LAUGH LAUGHS: Why bother? LAUGHS: There's no hope, Barbara. No. I` Uh, vagina tightening came in, uh, after this hotel closed. BOTH LAUGH I have no more words. I have no more words. BOTH LAUGH I have no more words either. Next on 20/20, a new and controversial procedure that's gaining popularity. Stay tuned. 25-year-old Akash Shukla and this 37-year-old man who asked not to be identified are part of a growing ` no pun intended ` but controversial trend in cosmetic surgery ` men who want and get their legs lengthened. Everybody is trying to alter what God gave them. If kids have crooked teeth, they'll get braces. But this isn't exactly like straightening your teeth. Do you hear that? The bone just broke. GENTLE LULLING MUSIC It's nice when someone's there to lend a helping hand. That's why Huggies Newborn Nappies have a wetness indicator that makes it easier to see when it may be change time and a waistband pocket to help protect against leaks at the back. Plus, they're the only nappy clinically proven to help prevent nappy rash. . Welcome back. So, here's one for the guys. Brotox. Yep, it's not just the girls lining up for cuts and tucks. The guys are in on it too. Also, a controversial new procedure ` leg lengthening. This one might make you squirm. Say goodbye to bellies and hello to Brotox. ROCK MUSIC Get it? Brotox? Because it turns out vanity knows no gender. It's becoming much more popular. It's been about a 100% increase since 1997. And why is that? First of all, the stigma of aesthetic treatments and surgery is becoming much much less. Reality TV with people being a lot more overt, it's becoming a little bit less taboo. But this isn't your father's plastic surgery. This new generation of cosmetic-enhancing guys want to look like themselves, just a little bit younger and some maybe even a little bit bigger. More on that in a minute. The popular guy procedures? Well, the top five are just like the women: liposuction, nose jobs, eyelid surgery, breast reduction and hair transplants done by robots. You want my good side? So all this got me thinking. How about a little nip and tuck here and there for moi? Sure, it's not the best lighting and it is one still frame in time, but as my grandmother used to say, there's no denying. Cutting to the chase, I suggest there isn't a single thing I would do. I love you, doc. But that said,... 'Uh-oh. Here it comes.' ...you would be a candidate for an eyelift. ...you would be a candidate for an eyelift. What would you do? Well, the answer would be tighten up the lower lid just a bit by removing a little bit of extra skin. You haven't mentioned my turkey neck. Well, we're not looking at this side of you yet. 'Ouch, doc.' How would you get rid of that? How would you get rid of that? The most appropriate place to actually put the seam, um, for the lower face and neck is here ` right in front of the ear and behind the ear. And pull it? And pull it? Undermining this area and pulling it up. Let me tell you, I have a trick of wiggling my ears. Watch this. If I freeze the ears back, it's a little like a facelift. Kind of bizarre, right? Wiggling the ears is a technique I haven't seen used many times, but you bringing your ears back isn't doing anything to your neck, so... That would be. That would be. (LAUGHS) So even though the scars wouldn't be visible, I think I'll go with the doc's original advice and not do anything. But if I did, here's what I'd spend. And insurance doesn't cover any of it. Expensive? Yes, but many men think of this as an investment because projecting confidence might just win you that girl or that promotion or maybe even... 'HAIL TO THE CHIEF' ...the Oval Office. Like it or not, looks are important, even in politics. Square jaw, bold eyebrows, smaller eyes ` experts say that conveys leadership. And it's not just the face. Size matters too. In more than half of all US presidential elections, the taller candidate went to the White House. This year it could be close. These two men aren't running for office, but they want to be taller, and they're making it happen. Before the surgery, when I was 4'11", people didn't take me seriously. Height discrimination is subconscious. Everyone does it to some degree. 25-year-old Akash Shukla and this 37-year-old man who asked not to be identified are part of a growing, no pun intended, but controversial trend in cosmetic surgery: men who want and get their legs lengthened. There are people that have said, 'Just accept what God gave you.' But everybody is trying to alter what God gave them. If kids have crooked teeth, they'll get braces. But this isn't exactly like straightening your teeth. Can you hear that? The bone broke. Dr Dror Paley, an orthopaedic surgeon at the Paley Institute at St Mary's Medical Centre in Florida, did 650 leg-lengthening surgeries last year. If you break a bone, pull it apart very slowly, like a millimetre a day, the bone knows how to grow new bone, and the tissues, like the muscle, nerves, the arteries, the skin, all of them regenerate. Most of Paley's patients have severe deformities or dwarfism, but increasingly these days his surgeries include cosmetic patients. He uses the state-of-the-art telescopic implantable rod. Once inserted into the middle of the broken bones, the rod actually expands and allows new bone to grow around it. Who knew? This is hardly a fast process. Argh! It takes at least three months to complete, including demanding and excruciating physical therapy. Ow! Ow! Who does this? The majority of patients who come for cosmetic limb lengthening have what we call, um, height dysphoria. They're unhappy with their height. They're unhappy with their height. How much does it really affect them? You've got a mild case, and you get people commit suicide. Wouldn't that lend itself more to some sort of psychologist? Therapy doesn't make a lot of difference. It's one of the few psychologic, psychiatric disorders that you can actually cure with the knife. Akash Shukla is now thrilled to be nearly 5'2", but this guy, who went from 5'6" to 5'9", well, that wasn't enough for him, so last fall he went back for more surgery to add another 5 inches. You know, I wanted the way I felt about myself and the way the world felt about me to be similar. So how does the world look at you differently at 6'1" than they did at 5'7"? I'll be 6'2" in four weeks. Um, I'm still lengthening right now. You're growing as we speak? You're growing as we speak? As we speak. He actually travelled overseas for the surgery, as many Americans do because leg lengthening might cost you about $85,000 with a Dr Paley, but you could pay less than half of that with a foreign doctor. But sometimes you get what you pay for. With these internal rods, the patient is the one who controls the lengthening, actually twisting their own legs to extend the rod inside. It makes a noise, so get ready to cringe. This is a click forward. POP! POP! This is a click back to reset. CRUNCH! CRUNCH! That is much quieter. That was one click. I have to do nine more of those today to do my 0.7mm. CRACKING CRACKING CONTINUES Was all this worth it? All this money, all this pain? Was all this worth it? All this money, all this pain? Absolutely. With women? With women? My dating life has improved exponentially. Because of those 4 inches? Because of those 4 inches? 6 inches so far. Oh, that clicking's horrible.
